What are the responsibilities and job description for the Full Time RN Admission Nurse position at Solaris Hospice?

Responsibilities:
- Performs admission and recertification assessments as delegated by the Clinical Director or designee.
- Provides professional nursing care by utilizing all elements of nursing process.
- Initiates the plan of care and makes necessary revisions as patient status and needs change.
- Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
- The Admissions Nurse submits Local Coverage Determinations as defined by Medicare and a Comprehensive nursing assessment to physicians, and obtains initial certification orders if not already obtained.
- Works in cooperation with the family/caregiver and hospice Interdisciplinary Team Members to meet the emotional needs of the patient and family/caregiver.
Qualifications and Requirements:
- Graduate of an accredited school of nursing.
- Current RN licensing, in good standing.
- Valid drivers license, and auto insurance
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.

Pay: $65,000.00 - $73,000.00 per year
