Organizational Summary: Soldier On is a private non-profit organization providing services for veterans that are homeless or at risk of becoming homeless. Soldier On operates over 120 beds of transitional and emergency beds in separate facilities for male and female veterans in Western Massachusetts. In 2010, Soldier On began developing permanent housing for veterans in Massachusetts and has over 300 permanent housing units in the state. Soldier On has partnered with developers to provide case management services in 36 units of permanent housing in upstate New York and has been awarded funding through the Empire State Supportive Housing Initiative (ESSHI). In New Jersey, Soldier On operates over 70 units of permanent housing. Soldier On has been the recipient of three SSVF grants since September 2011. The SSVF program is a grant funded by the VA awarded to private non-profit organizations and consumer cooperatives. These grants provide case management services, outreach services, housing assistance, healthcare navigation, and temporary financial assistance for veterans that are homeless or at risk of becoming homeless.
Position Summary:
The incumbent will conduct assessments for eligibility and needs with veterans and in conjunction with the client develop an individualized plan through a housing-first approach. The position is responsible for the coordination and implementation of supportive services to veteran and/or veteran’s household, with the goal of services being long-term independent housing stability. The case manager will provide appropriate referrals and assist the client in accessing services related to their housing stability. The incumbent will offer housing counseling and information on tenant’s rights. The majority of job activities are performed in the community in Cape May County and surrounding counties; at shelters, social service agencies, veteran’s apartments, and other locations deemed appropriate through a mobile case management model. The position reports to the Field Supervisor.
Qualifications:
· Relevant education and/or experience in human services or related field. Bachelor’s degree required.
· Valid driver’s license required.
· Ability to drive up to 6 hours per day if necessary.
· Ability to be supportive and respectful to all veterans and household members served.
· Ability to work in a variety of settings with culturally diverse families.
· Ability to be culturally sensitive and appropriate.
· Capability to work both independently and as part of team.
· Ability to collaborate with in-house and external referral sources.
· Possess strong written and oral communication skills.
· Demonstrate a strong level of understanding and comfort with various forms of technology including data entry and HMIS systems.
· Ability to take direction and participate in weekly supervision.
· Preference given to candidates who are veterans and/or have lived experience with homelessness or housing instability.
Responsibilities:
· Conduct eligibility and intake assessments.
· Assess functional and social needs of the veteran to co-create an individualized housing stabilization plan with the veteran.
· Develop and maintain relationships with community service providers.
· Determine and access appropriate Veterans Administration, community, state, and federal benefits and resources.
· Provide case management and follow-up services through a lens of progressive engagement.
· Conduct outreach in a variety of settings including, on the streets, in encampments, shelters, hotels/motels, and other places not meant for human habitation.
· Coordinate referrals and follow up on outcomes.
· Maintain client confidentiality.
· Provide support services in crisis situations appropriately and quickly.
· Assist veterans in obtaining and maintaining permanent housing.
· Document and maintain accurate information in electronic client files, including progress notes on all visits and contact, on a regular basis.
· Knowledge and understanding of company guidelines.
· Adhere to agency policy, procedures, and professional code of ethics.
· And other duties assigned by supervisor.

Pay: $55,000.00 per year
