Job Description

Job Title : Licensed Practical Nurse (LPN) - Full Time Nights

Job Specialty : Long-Term Care (LTC)

Job Duration : 13 weeks

Shift : 11pm-7am, 5x8 nights per week, every other weekend and holidays required

Guaranteed Hours : 40 hours per week

Experience : 1 year of experience as an LPN

License : LPN Licensure

Certifications : BLS (through AHA or RQI specifically)

Must-Have :

1 year of experience serving in an LPN capacity in a long-term care facility

Job Description :

Demonstrate the knowledge and skills necessary to provide care appropriate to the age of patients served.

Demonstrate knowledge of the principles of growth and development of the life span.

Assess and interpret data reflective of the patient's status and provide age-specific care.

Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.

Maintain collaborative relationships with nursing staff, physicians, and ancillary staff.

Maintain knowledge and clinical expertise through continuing education and in-service programs.

Collect data relevant to residents' physical and psychosocial conditions and communicate findings to the nursing team.

Contribute to a culturally relevant plan of resident care, promoting a healthy, safe environment.

Recommend revisions to the plan of care after evaluating resident outcomes and facilitate referrals as needed.

Collaborate with the resident / family and interdisciplinary team within the scope of nursing practice ensuring satisfaction.

Participate in mentoring and orientation of staff.

Consider safety, cost, and impact in care planning and delivery.

Provide leadership in the professional practice setting under the direction of an RN.

Maintain competency and knowledge in nursing practice, considering occupational exposure to blood-borne pathogens.
