Job Description :
- Provide direct and indirect resident care under the direction of licensed nursing personnel within the scope of practice.
- Meet resident's activities of daily living (ADL) needs as outlined in the plan of care, ensuring a safe and comfortable environment, including hygiene, grooming, dressing, incontinence care, toileting, positioning, and skin care. Complete documentation as required.
- Maintain resident's environment and safety as per the care plan, including promptly answering call lights, providing safety equipment, and maintaining cleanliness. Report and document any malfunctions in equipment.
- Fulfill resident's nutritional needs according to the care plan, honoring preferences and choices. This includes meal delivery and assistance with feeding. Complete documentation as required.
- Support resident's rehabilitative needs as per the care plan, including range of motion, ambulation, transfers, and use of assistive devices. Complete documentation as required.
- Report resident's rights, refusals, and concerns to the Charge Nurse promptly, and identify any changes in physical, emotional, or psychosocial status.
- Be aware of potential exposure to bloodborne pathogens and take necessary precautions during tasks involving contact with blood, body fluids, or tissues.
