POSITION SUMMARY:  

The Case Manager is responsible for providing wrap around services to victims of crime clients and their families and will be involved in all aspects of the program including intake, case management, and making appropriate referrals to services. 

DUTIES AND RESPONSIBILITIES:  

• Meet with clients at bedside at Los Angeles County Hospital until discharge

• Assess client readiness for case management services based on needs, eligibility and ability and willingness to participate in program

• Create case plans for clients with short, medium, and long-term goals and the steps to achieve those goals

• Meet at the client's home and office as needed to identify the client’s needs and resources available to the individual and family

• Provide direct services and referral services, including but not limited to, neighborhood-based case management, relocation and transition, educational support, housing, mental health, substance abuse treatment, tattoo removal, etc.

• Follow up with clients that are discharged to ensure services are being met and provided

• Evaluate clients’ progress periodically and making adjustments as needed to improve outcomes

• Assist in coordinating and leading client groups for pro-social activities

• Provide individual coaching to guide the clients through successes and challenges

• Establish and maintain good working relationships with CBO’s, schools, probation and/or and stakeholders working with clients

• Maintain accurate and detailed case notes regarding client’s progress or any relevant information to the case planning

• Complete necessary documentation, including: community engagement, daily activities, tracking services, and any other data collection required by the contract for service delivery 

• Maintain confidentiality and comprehensive confidential case files

• Prepare monthly case-related reports including outcomes, successes and challenges

• Advocate for clients with agencies and service providers to minimize barriers to clients receiving needed services

• Actively participate in staff meetings, trainings, client reviews, internally and with partner agencies

• Establish and maintain collaborative working relationships with community resources 

• Performs related duties as required. *

REQUIREMENTS

1. Fully vaccinated for COVID-19 as outlined in the contract requirements

2. Comply with Hospital Background/Clearance as outlined in the contract requirements

Qualifications and Experience

Any combination of experience and education likely to provide the required knowledge, skills and abilities, typically:  

• Bachelor’s degree in social work, mental health or a related field  
• 1-2 years of experience providing direct field-based case management services
• 1 year of experience working with People Who Use Drugs (PWUD), People Experiencing Homelessness (PEH) and/or who have chronic health issues (preferred)   
• Commitment to the mission and values of the non-profit sector, with a passion for social impact and community service
• Must have valid CA Driver’s License, automobile, and proof of auto insurance 
• Must be able to successfully pass Live Scan (background check)
• Required TB Test clearance
• Required Hospital Clearance Process

Salary : $50,000 - $55,000
