Work from Home MTM Pharmacist Job- Atlanta GA

Soliant is currently seeking a pharmacist with two or more licenses in NJ, LA, GA, KY, TX, and UT for a contract position. This opportunity offers remote work, providing medication adherence/management calls with direct patient outreach. The ideal candidate will have a strong background in Medication Therapy Management (MTM), with experience in health plan or pharmacy benefit management (PBM) settings preferred.

Responsibilities:
- Conduct medication adherence/management calls, addressing elevated inquiries that require pharmacist intervention.
- Prevent, detect, and resolve medication-related problems, making evidence-based, patient-centered remendations.
- Perform thorough medication reconciliations for post-discharge patients.
- Assist in the management of high-risk, high-cost patients to ensure optimal care.
- Conduct telephonic prehensive medication reviews (CMRs) for qualified patients.
- Identify and address patient-specific barriers to medication adherence and disease management.
- Provide clinical decision support, education, and consults to providers as needed.
- Coordinate care between care managers, providers, and pharmacies to ensure appropriate medication management.
- Collaborate with providers and staff to optimize medication management workflows and enhance patient experiences.
- Adjust and remend alternative pharmacotherapy based on formulary changes.
- Follow evidence-based guidelines for managing chronic disease states, collaborating with providers as required by state laws.

Experience:
- Strong background in Medication Therapy Management (MTM) required.
- Experience in health plan or pharmacy benefit management (PBM) settings preferred.
- Retail pharmacists with MTM experience will also be considered.

Hours:Monday - Friday, 9:00 am to 5:00 pm EST (hours may expand)
