Home-based Family Case Manager
Solid Rock Family Services is looking to add a youth and family Case Manager to join our team. We are committed to providing evidence-based, innovative, and culturally sensitive services that will foster transformative growth for youth and their families in Marion County and surrounding areas. As a Case Manager at Solid Rock Family Services, you will be providing counseling services that will enhance family relationships, build connections with community services, and provide hope to youth, families, and the community.
Position Overview
- Contract Position
- Paid Monthly
- Carry caseload of 8 - 10 families
- Complete Family assessment within first 3 meetings
- Develop and document Treatment plan in conjunction with family
- Develop Safety Plan with family sign-off
- Attend client Team Meetings as requested
- Attend Court as requested by FCM
- Hold family sessions with each family as agreed, scheduled at the convenience of the families
- Attend weekly staffing supervision meetings
- Documentation of all meetings, interactions and events with caseload within 48 hours of occurrence
- Complete monthly visitation and casework reports detailing activities of the month including progress updates, next steps and goal attainment to date.
- Monthly reports must be completed by the 5th of the following month.
- Provide on-call availability to families in times of crisis
- Drive throughout the city to meet with families in the home or community in which they live.
- Case Managers provide services in families' homes in a wide array of settings and communities
- Conduct supervised visits at our facility, in the clients' homes, or at community locations as appropriate.
Additional Information
Schedule is flexible and non-traditional; based around the availability of youth and families served. Evenings and weekends are included.
Applicants must have a valid driver’s license, a reliable personal vehicle to use for work purposes and auto insurance.
Qualifications
Education
- A Bachelors degree in a social services discipline is required for a Case Manager
- Degrees that can be considered include social work, counseling, psychology, or marriage and family counseling.
- Other degrees will have to be evaluated further to determine if they meet the qualifications.
Requirements
- Strong organizational skills and attention to detail
- Excellent written and verbal communication skills with a focus on clear, concise and grammatically correct documentation and report writing.
- Ability to communicate effectively across varied cultural, economic, social groups.
- Ability to manage multiple priorities simultaneously
- Computer knowledge including Microsoft Word, Gmail, Adobe Acrobat, and Dropbox,
- Ability to maintain a flexible schedule.
- You will be expected to learn, through In-service training, follow and implement policies and procedures, standards and expectations set in place by the Department of Child Services of Indiana and the Agency itself.
