Wellness Coordinator Responsibilities:
1. Provides and coordinates caregiver training as needed to comply with company policy. Ensures care staff is trained in the care needs necessary to care for residents by completing skills checklists.
2. Responsible for preparing and setting Heath Services schedules to meet the needs of the community.
3. Observes and reports residents for changes in behavior, health or signs of abuse/neglect and notifies Wellness Director and ED.
4. Participates in all scheduled team member meetings pertaining to assisted living residents or staff as well as voluntary and mandatory on-going education and in-service training.
5. Performs data entry into electronic care program to updates resident information, resident physical and emotional condition changes, and vitals, using appropriate terminology and abbreviations.
6. Maintains 100% compliance with current ADHS Rules and requirements.
7. Coordinates resident care with Wellness Director and caregivers.
8. Oversees resident medications second to WD when on duty for med exceptions and takes appropriate action.
9. Performs and completes assignments within established procedures and guidelines.
10. Conducts assessments as required in the absence of the Wellness Director.
11. Schedules appointments as needed.
12. Be in on call rotation, cover shifts when needed.
13. Order resident supplies.
14. Interviewing potential staff as needed.
15. Perform TB skin tests to maintain compliance.
16. Direct and supervise caregiver personnel. Direct day to day functions of caregivers in accordance with current rules, regulations and guidelines that govern assisted living care.
Wellness Coordinator will report to Wellness Director.
The schedule is Tuesday through Saturday.

Qualifications
- An equivalent combination of experience, education, and other qualifications may be considered.
- Valid LPN license.
- Current Fingerprint.
- Current CPR/First Aide
- Current TB
- Current Food Handlers
- Able to lift a minimum of 25 to 75 pounds.
- Tuesday through Saturday Schedule.
- Reports to Wellness Director.
SKILLS:
- LPN Required
- Must be able to read, write, and speak fluent English.
- Must have excellent observation, good nursing judgment, and good oral and written communication skills.
- Professional in attitude, demeanor and dress.
- Good organizational skills and ability to work under pressure and meet deadlines.
- Ability to be flexible in working hours.
- Ability to be on-call.
