Position Overview:
We are seeking a dedicated and experienced Family Nurse Practitioner (FNP) to join our team. The FNP will provide comprehensive primary care services to individuals and families across all age groups. The ideal candidate will possess strong clinical skills, a commitment to patient-centered care, and the ability to work collaboratively with a multidisciplinary healthcare team.
Key Responsibilities:
- Conduct comprehensive health assessments, including history taking, physical examination, and appropriate diagnostic testing.
- Develop and implement individualized treatment plans based on patient needs, evidence-based guidelines, and best practices.
- Provide preventive care, health education, and counseling to promote wellness and healthy lifestyles.
- Diagnose and manage acute and chronic conditions such as diabetes, hypertension, and respiratory issues.
- Prescribe medications and monitor their efficacy and potential side effects.
- Collaborate with physicians, specialists, and other healthcare providers to ensure coordinated patient care.
- Document patient encounters, assessments, and treatment plans in electronic health records (EHR) accurately and timely.
- Perform minor procedures, such as wound care, suturing, and joint injections, as required.
- Educate patients and their families about managing conditions, medications, and preventive care strategies.
Qualifications:
- Master's or Doctorate degree in Nursing from an accredited program.
- Current certification as a Family Nurse Practitioner (FNP) through the ANCC or AANP.
- Active and unrestricted Arizona Nurse Practitioner License.
- Strong communication, clinical assessment, and decision-making skills.
- Ability to build rapport with patients and collaborate effectively with healthcare team members.
Preferred Skills:
- Experience working with diverse populations.
