We are seeking a highly skilled Nurse Care Coordinator to join our team at Solutions by Sanford. As a Nurse Care Coordinator, you will play a vital role in delivering high-quality patient care and coordinating care transitions across the continuum.
About the Role

Key Responsibilities
- Assess patient needs and develop individualized care plans using the nursing process.
- Collaborate with inter-professional colleagues to plan, implement, and evaluate care.
- Coordinate care transitions across the continuum and facilitate access to necessary resources and services.
- Care for patients in all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up.
Qualifications and Requirements
- Graduate from an accredited nursing program required, including American Association of Colleges of Nursing (AACN), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
- Bachelor's Degree in nursing preferred.
- Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
