General Statement of Duties :
Completes Choices for Care assessments for various programs and services including; homemaker, adult day services, assisted living, and nursing facility placement. Provides planning, problem solving, benefit linkage and referral to local resources. Promote problem solving, family system integrity and client independence. All assessments are conducted at hospitals or nursing homes in the Berwyn, Cicero and North Proviso areas.
Qualifications :
A Bachelor degree in Sociology, Social Work, Gerontology or related field is required. One year of experience working with the elderly is preferred. Candidates must demonstrate strong organizational, time management and communication skills, computer literacy and the ability to work with the elderly. Bi-lingual in Spanish is preferred but not required. Familiar with medical terminology is a preferred, but not required. Local travel to trainings and meetings also required. Must have own car and insurance. Weekend hours are required. Criminal background required.
Requirements :
Must demonstrate strong organizational and time management skills and be adaptable to changing work conditions. Strong communication skills, the ability to work with diverse populations and proficiency with Microsoft Office (Word, Excel, and Outlook) to maintain accurate case notes, reports and record keeping are required. Must be comfortable multi-tasking on the computer using different screens. Availability of a vehicle for local travel to hospitals, trainings, and meetings is required. Valid driver’s license without restrictions is required. Criminal background check required.
Training :
Must be able to attend and pass the state mandated Illinois Department on Aging training which will require attending training in Springfield, Lombard or Chicago, Illinois.

As a condition of employment, employees, interns, and onsite volunteers of Solutions for Care must be fully vaccinated with (or in process of) an approved COVID-19 vaccination. Must show proof of full vaccination on first day of employment.
Duties and Responsibilities :
1. Develop excellent knowledge of programs, benefits and services available to seniors on a local, regional, state and federal level.
2. Gain and understanding of program policies and procedures.
3. Meet all funding source training and certification requirements for this position.
4. Manage assigned clients including accurate and timely documentation of all interviews for each client – including case note entry, data reports and record keeping.
5. Advocate for the client and assist in problem solving, family system intervention, benefit assistance and support of clients’ rights and choices.
6. Will complete approximately 100 patient assessments per month.
7. Maintain written weekly schedule as required by supervisor.

- Must be comfortable conducting visits at hospitals, nursing facility, supportive living and rehabilitation center, etc.

Schedule :
Full time : 8 : 30 am to 4 : 30 pm, Monday through Friday and Rotating weekends.
Salary : $40,000. During the first year, six-month and 12-month performance reviews are conducted. Thereafter, performance reviews will be conducted every 12 months. Salary increases are based on performance.
