Job Title : Pharmacist (Full-Time Position)
Location : Baltimore, MD VA Medical Center (VAMC)
Employer : Solvet, working in collaboration with the Baltimore VA Medical Center

Overview :

We are seeking dedicated and qualified pharmacists to become an essential part of our team. As a Solvet-employed pharmacist, you will have the opportunity to support veterans' healthcare needs in both inpatient and outpatient settings, collaborating with a dynamic healthcare team to ensure optimal medication therapy outcomes.
HOURS :
Required to work a minimum of 40 hours per week per pharmacy technician with each shift being a minimum of 8-hours.(Sunday through Saturday, 24 hours a day, 7 days a week, including holidays). This may include but is not limited to a 7-days on and 7- days off.
Key Responsibilities :
As a pharmacist at the Baltimore VAMC, your responsibilities will include, but are not limited to :
Centralized Unit Dose Program :
Review and verify medication orders to ensure appropriateness, correct dosages, and absence of harmful drug interactions.
- Provide therapeutic alternatives and consult with physicians to resolve medication-related issues.
- Conduct discharge counseling with patients, educating them on their drug regimens and potential side effects.
Centralized Intravenous Admixture Program :
Prepare intravenous admixtures, including chemotherapy and total parenteral nutrition (TPN) solutions, adhering to USP 797 / 800 standards.
Outpatient Activities - Prescription Order Processing & Counseling :
Review outpatient prescriptions to ensure accurate medication dispensing and assess for drug interactions.
Clinical Services :
Monitor drug therapy outcomes through physical assessments and diagnostic data, making recommendations as necessary.
Customer Support & Vaccine Administration :
Deliver excellent customer service, responding to inquiries via calls, emails, and other communications in a timely and professional manner.
Requirements :
OR, if a graduate of a foreign pharmacy program, must have obtained FPGEC Certification.
Preferred : At least 2 years of experience in hospital (inpatient) pharmacy.
