Position Summary:
Provides mental health assessment and treatment collaboratively with individuals though wellness and recovery, person-centered care to patients. Serve as consultant to other Center staff. Participate in Performance Improvement (PI), peer review and medical staff activities. Assesses individuals with mental health issues in a clinical setting, records medical history, creates treatment plans, and prescribes through a shared decision-making process together with individuals. Supervised by Medical Director. Provides clinical coverage as needed and as appropriate based on the needs of the Center. Supervises student interns.
Essential Functions:
- Perform psychiatric evaluations, including history and mental status exam.
- Complete medication management treatment plans with each individual in a shared decision-making process, and in collaboration with other clinical staff.
- Prescribe medication and monitor its benefits/side effects in a shared decision-making process with each patient served.
- Respond to inquiries directed by staff and nurses.
- Communicate on a regular basis with Center staff, families, outside agencies.
- Perform PI, peer review, committee assignments as necessary.
- Supervises student interns
- Maintain thorough, timely clinical documentation
- Provide clinical coverage as needed and as appropriate based on the needs of the organization
- Performs other duties as assigned
Essential Qualifications:
- APN license by the State of New Jersey
- Certified Advanced Registered Nurse-Advanced Practice certification
- Three (3) years’ of experience with individual with severe and persistent mental illness and co-occurring disorder, 2 of which are in addictions; minimum 1 year of working with culturally diverse/at-risk populations
- Three (3) years’ experience working with adults with complex health needs and LGBTQ persons across various domains of care to address disabling or life-threatening issues, prevent adverse health outcomes and increase accessibility to care for persons of diverse cultures and languages.
- Age Specific Knowledge & Skills necessary to provide care to: Children, Adolescents, Adults, Older Adults
- Must demonstrate knowledge of key principles of trauma-informed, person-centered wellness and recovery delivery of services, growth, and development across the life span
- Possess the ability to assess data reflective of the individuals’ status in accordance with their goals and be able to interpret the appropriate information needed to identify each individuals’ needs relative to his/her age in accordance with the Center’s policies and procedures.
- Experience treating a variety of mental health issues in adult patients.
- Computer Literacy/skills.
- Proficient at working with Electronic Medical Record
Preferred Qualifications:
- Bilingual English/Spanish
- BCLS Certification
