How You’ll Make an Impact
- Build trusting, compassionate relationships with patients and their families
- Empower caregivers through education and support
- Collaborate with an interdisciplinary team to provide comfort, peace, and dignity
- Coordinate and oversee all aspects of in-home patient care
What You'll Do
- Develop, implement, and evaluate individualized care plans
- Perform head-to-toe physical assessments and vital sign checks
- Administer medications, injections, IV therapy, and tube feedings
- Manage catheters and other medical devices
- Educate patients and families about care needs and expectations
- Supervise Home Health Aides (HHAs)
- Accurately complete timely documentation
- Communicate regularly with the clinical manager, physicians, and the care team
- Participate in the on-call rotation and be available for evening/weekend visits as needed
- Other duties as assigned
What You’ll Need
- Current RN license in the state of Texas
- Minimum of 2 years of RN experience (hospice experience preferred)
- Current CPR and BCLS certification
- Valid driver’s license and reliable transportation
- Excellent verbal and written communication skills
