Responsibilities
- Conduct comprehensive physical examinations and health assessments of patients.
- Diagnose and manage acute and chronic health conditions.
- Develop, implement, and evaluate individualized treatment plans.
- Perform medical procedures including catheterization, suturing, spinal taps, and medical imaging as needed.
- Provide education to patients and families regarding health management and disease prevention.
- Collaborate with interdisciplinary teams to ensure effective discharge planning for patients.
- Maintain accurate patient records and documentation in compliance with ICD-10 coding standards.
- Provide specialized care for patients with dementia and critical care needs.
- Manage Durable Medical Equipment (DME) requirements for patients as necessary.
Skills
- Strong knowledge of physiology and anatomy relevant to patient care.
- Proficiency in performing catheterization, suturing, spinal taps, and other medical procedures.
- Familiarity with medical imaging techniques and interpretation of results.
- Ability to develop effective discharge plans tailored to individual patient needs.
- Experience in dementia care and managing critical care situations is highly desirable.
- Excellent communication skills for patient education and collaboration with healthcare teams.
- Strong organizational skills to manage multiple patient cases efficiently.
