Responsibilities:
- Provide comprehensive care coordination, case management, and care management services.
- Develop and implement disease management programs, educating and motivating members.
- Offer triage and referral, transition of care planning, and end-of-life care planning.
- Provide nurse education and assistance with managing daily functional needs.
- Help members navigate health plan benefits and access alternative services.
- Manage medical expenses within the health plan framework.
- Continuously work to improve access to and quality of care.
Required Qualifications:
- 5 years of clinical nursing experience.
- Experience collaborating with physicians on patient care management.
- Current, unencumbered RN license in your state of residence (WA or OR).
Preferred Qualifications:
- Bachelor's Degree in Nursing or Health Education.
- Current, unencumbered RN licenses in both Oregon and Washington (within six months of hire).
- Experience in Case Management and/or Utilization Management.
- Experience working in a health plan or similar environment.
- Current nursing experience in relevant areas (e.g., chronic pain management, cardiology, etc.).

Compensation:
