The Behavioral Health Counselor is responsible for providing direct behavioral health services in home, clinic, and field setting to adults and older adults. This position provides services that include providing individual and group therapy and performing case management services. The Behavioral Health Counselor delivers services with attention to cultural relevance and sensitivity.

Responsibilities:
- Provide culturally sensitive, client-centered individual, group, and family counseling and rehabilitation services to adults and older adults.
- Facilitate referrals to community resources and performs case management services.
- Develop culturally relevant assessment and treatment plans.
- Complete all documentation per Medi-Cal, Santa Clara County standards within expected deadlines.
- Participate in clinical supervisions, consultations, team meetings, trainings, and after hours rotation.

- Requirements:
Master’s degree in social work or psychology; or equivalent education, training, and experience.
- One year or more of direct clinical experience working with adults.
- This job requires you to be in the community, in client’s homes, and other community partner sites. Driving is essential to the function of this job. The job requires travel up to 25% of the time.
Salary : $75,000 - $82,000
