Case Management / Discharge Planning Utilization Review2 yrs expLocal Candidates accepted at a lower rateMust have at least 2 years DISCHARGE PLANNING EXPERIENCE in a Hospital Acute Care setting covering a vaiety of adult units. ER experience is a plusExperience must include A MINIMUM of 6 mos at a level 1 or large academic Medical Center hospital working with a variety of inpatient units for medical discharge planningEPIC CHARTING experience is REQUIRED
