Qualifications:
- Must have a high school diploma (HS) or GED.
- Strong understanding of healthcare delivery systems and the importance of integrated care.
- Experience or familiarity with care coordination processes, including managing transitions between providers and services.
- Proficiency in health IT systems is a plus, particularly as it relates to supporting care coordination activities.
- Additional certification in case management or related fields is highly desirable.
Responsibilities:

- Assist consumers and families in understanding and navigating the healthcare system, including the use of health IT systems when appropriate.
