Base Salary Range: $131,461 - $152,315
FTE: Part-time, Non -Exempt

General Summary: As the ECM Clinical Care Manager, you will play a pivotal role in overseeing and coordinating ECM process and care management activities, ensuring the delivery of effective and efficient healthcare services to our patients. This role requires a combination of clinical expertise, patient advocacy, and a passion for improving patient outcomes. The ideal candidate will have to perform some of the following essential duties.
- Coordinate and facilitate care for patients with complex medical needs, ensuring that care plans are tailored to the patient needs and preferences, in doing so a seamless transition between healthcare providers and settings.
- Advocate for patients' needs and collaborate with the interdisciplinary healthcare team to address barriers to care and improve patient outcomes, to ensure all necessary components, (goals & interventions) are included.
- Collaborate with the ECM Administrator to develop and implement initiatives that enhance the quality and efficiency of care management services.
- Ensure adherence to clinical guidelines, regulatory requirements, and organizational policies. Ensure care plan assesses the member's health status and ensure the care plan is member specific, addresses their needs specific to their population of focus, and addresses all?care gaps identified in the assessment. Care plan goals should be written in SMART format.
- Can provide guidance in terms of care gaps and where to refer patient to in order to support care gaps identified through the assessment.
- Provide constructive feedback to discuss strengths and areas for improvement in each care plan, in order to recommend specific changes or additions to enhance care plans based on best practices.
- Ensure accurate and timely documentation of client interactions in the NextGen Electronic Health Record (EHR) system, maintaining compliance with LA Care, Health Net and Molina Healthcare reporting standards.
- Source, create, and disseminate health promotion materials for Lead Care Managers to distribute to program members. Collaborate with internal and external stakeholders to ensure materials align with evidence-based practices and cultural competence.
- Conduct weekly clinic conference with the Lead Care Manager’s to discuss patient cases internally
- Ensures that everyone actively involved in the care plan (including patient) is scheduled to discuss case updates and next steps.
- Willingness and ability to manage caseload (1-10) of high needs clients as needed.
- Bachelor’s degree in nursing, social work, or a related field. Master’s degree preferred.
- Valid Registered Nurse (RN, FNP) license in California, Psychiatric Mental Health Nurse Practitioner (PMH-NP), PMH Clinical Nurse Specialist (PMH-CNS), Advanced Practice Nurse, Nutritionist or Social Worker.
- Knowledge of or willingness to learn LA Care, Health Net, and Molina Healthcare reporting requirements for clinical oversight.
- Current California Driver’s License and Automobile Limited Liability Insurance.
- Must have a minimum 1-3 years’ experience in a community clinic or FQHC setting
- Experience creating or assisting with Care Plans for high-needs clients.
- Experience working with Limited English individuals in a small group format.
- Must be able to communicate effectively, in English, both verbally and written.
- Demonstrated ability to act as liaison between SCFHC staff, and patients.
- Demonstrated ability to handle highly confidential information.
- Ability to work flexible hours.
- Comfortable working with diverse populations
