Job Summary
Responsible for providing compassionate and comprehensive care to patients in a home health setting. Assess patient needs, develop care plans, and administer medications.
Essential Duties and Responsibilities
- Professional Responsibilities:
- Assess patient needs and render appropriate care.
- Perform patient teaching, functions as a member of the health care team in delivering mission-driven, quality patient care.
- Attend all in-services, education programs and scheduled meetings as outlined by the Director of Nursing/Home Health Director.
- Consistently demonstrate proper nursing judgment, deliver patient care within the scope of service for RN.
- Maintain and demonstrate constant awareness of State and Federal Regulations pertaining to Home Health care.
- Core Duties:
- Demonstrate performance standards
- Appropriately evaluate signs and symptoms of disease processes, reactions to treatments, medication and effectiveness of teaching.
- Deliver age-appropriate treatment according to the patient’s plan of care
- Complete a thorough assessment of the patient’s pain and intervene appropriately
- Communicate appropriately and respectfully the nursing process and treatment according to the patient/caregiver ability to comprehend
- Develop and document appropriate goals
- Follow agency policy regarding discharge planning: complete necessary discharge information (D/C Summary)
- Communicate and document patient updates to physicians, other members of the health care team and to the patient’s medical record timely
- Develop, implement and update nursing goal/patient outcomes and aide care plan as necessary.
- Supervise home health aide to ensure quality patient care and update care plan as needed.
- Complete patient scheduling and notify patient/caretaker and staff of any changes in an appropriate and timely manner
- Appropriately evaluate signs and symptoms of disease processes, reactions to treatment, medications and effectiveness of teaching.
- Follow agency policy regarding discharge planning, complete necessary discharge information (D/C Summary/OASIS)
- Follow orders given by provider
- Perform other job duties and tasks assigned
Qualifications
Education and Experience
- Graduate of an accredited school of nursing
- Current Mississippi RN license
- CPR certification
Preferred Qualifications
- Prior Home Health and/or Med Surg floor experience
Minimum Requirements
Essential Skill Competencies:
- Body system assessment
- Phlebotomy: Establish IV access
- Safe blood administration
- Medication monitoring
- Preventive skin care
- Chest tube care
- Aseptic wound care/ dressing change
- Foley catheter insertion/care
Working Conditions
Ability to sit, bend, stoop, kneel, crouch, perform overhead lifting and perform other common physical movements as needed for the position.
