Position Objective/Summary: Provides case management services for an assigned caseload. Works closely with consumers and/or their families. Communicates with staff, vendors and the community. Organizes and resolves work-related situations.
Minimum Qualification Required For This Position: Bachelor's degree (B.A.) from a four-year college or university; two years related experience and/or training; or equivalent combination of education and experience. Internships and/or training level employee may have less requirements. Must have two years of early intervention experience 0-3 years of age.
General Statement of Functions: Contacts assigned consumers and documents; contacts according to established SCLRC Standard Operating Procedures (SOPs) and policy, and completes required documentation. Prepares case transfers as needed. Attends all unit, division, supervision, staff and in-service training meetings as required. Attends all Planning Team meetings at primary programs and any other consumer related special meetings/hearings (e.g. SCAN, court, IEPs/IFSPs staffing's, mental health, governmental/community agencies, placement, etc.) as required. Handles offer-of-the-day (OD) duties as required. Handles confidential information and adheres to Welfare and Institution (W&I) codes. Consults with appropriate specialists regarding the service needs of consumers. Maintains effective working relationships with top management, peers and staff. May act as Program Manager in his/her absence.
