Gulf Coast Social Services is currently seeking experienced Clinic Case Managers for South Central Louisiana Human Services Authority (SCLHSA) for its Terrebonne office and surrounding areas.
The Clinic Case Managers provide clinical case management activities across the continuum of care from assessing and planning to implementing, coordinating, monitoring and evaluating individuals and families in Houma, Louisiana. This position also involves meeting assigned clinic goals and objectives, using appropriate methods, as outlined by clinic supervisor. The position meets regularly with clients and must also prepare reports documenting progression or regression of the clients, as well as maintain case records and other duties as directed by the supervisor.
This position requires a Master’s degree or a Bachelor’s degree in a human service related field, with a minimum of one year relevant experience. The position also requires organizational, verbal and written communications skills. Experience with MS Office, including Word, and Excel is also desired.
Applicant must pass Criminal Record Background Check and LA Motor Vehicle Record Check, have a valid driver's license consistent with Louisiana law, current auto liability insurance, and have access to a reliable vehicle in a safe operating condition with current inspection stickers. EOE
