SCMHCC is currently recruiting for a full-time Wraparound Facilitator. GENERAL RESPONSIBILITIES:
Coordinate care for clients and families to prevent hospitalization or out-of-home placement and to reintegrate consumers into the community. Develop rapport with targeted populations to link caregiver(s) with resources. Deliver services within the Community Based Program to children diagnosed with serious emotional disturbances.
Judgment: Sound judgment is used in ongoing assignments. Maintains a positive, professional and supportive relationship with consumers. Maintains a positive and professional relationship with staff from SCMH and other agencies. Confidentiality is maintained at all times. Specialists keep other staff informed of unusual situations regarding consumers.
Documentation: Progress notes are accurate and reflect progress toward treatment goals. Documentation is current and turned in weekly.
Essential Job Functions:
- Develop continuous supportive relationships with identified clients and their families.
- Collaborate with family, MCO, and wraparound team members to assemble and facilitate client’s initial, quarterly, and annual SED Waiver treatment reviews. Assist family in identifying natural community supports to be part of client’s wrapround team. This meeting needs to include the development of goals, objectives, services, strengths and needs assessment, and crisis plan.
- Update client’s goals, objectives, and services in the treatment plan if needed and follow-up with the therapist for their clinical oversight. Follow-up with other treatment team members for progress updates.
- Communicate with client’s MCO regarding medically necessary SED Waiver services and authorizations needed to meet client’s mental health needs.
- Educate family on SED Waiver services, rules, and guidelines; including ensuring client is utilizing at least one SED waiver service each month.
- Facilitate client autonomy by helping them build on strengths, increase community integration, and learn more effective controls for emotional disturbances based on medical necessity.
- Coordinate with client’s QMHP during client’s Annual Review period to schedule ALOC/CAFAS interview. Coordinate with family and wraparound team to ensure Annual POC is completed timely as dictated by SED Waiver policy.
- Assist client and family with utilization of natural community supports and safety planning when transitioning client off SED Waiver services when no longer functionally eligible.
- Facilitate client autonomy by helping them build on strengths, increase community integration and learn more effective controls for emotional disturbances based on medical necessity.
- Build a relationship with community agencies that will provide a natural, supportive network for clients in their quest for mastery in various situations.
- Assume an advocacy role for clients and their families.
- Have a minimum of 60% of work hours of client contact per week.
- Complete check-ins with families at minimum every 45 days to ensure medical services are being provided and help bridge any barriers to services.
- When necessary or possible, help integrate clients into existing emergency service systems.
- Perform other duties as directed by supervisor.

Minimum Qualifications:
Have at least a BA/BS degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substitution for one year of education. Must have the ability to communicate positively and effectively with others, and to work independently as well as be a productive part of a team. Must have a dependable vehicle and valid driver’s license. Must provide proof of insurance. Pass a criminal background and driver’s license check to the satisfaction of SCMH.
