The Case Manager will manage a caseload of clients experiencing a first episode of psychosis through the activities of planning, linking, monitoring advocacy and assessment.
Duties & Responsibilities
- Maintain a caseload of 25-30 clients
- Complete ongoing Needs and Risks Assessments with clients
- Coordinate care with the team as well as external services
- Collect vital signs prior to medication management appointments
- Link clients with community resources
- Advocate and accompany clients to community resources
- Bi-monthly client contact with each client on caseload
- Monitor treatment process to ensure needed services are provided in a timely manner
- Monitor treatment progress to ensure proper discharge and transfer of needed services is arranged/recommended
- Maintain records including progress notes and routine assessments in a timely manner in the Electronic Health Record (EHR) platform.
- Participate in the collaborative treatment planning process
- Participate in weekly team meetings
- Receive monthly individual program supervision for one hour provided by the Director
- Complete staff development activities
- Special projects and duties as assigned
- BA/BS in Human Services field from an accredited college or university
- 1 year or more on Mental Health field
- Trainings, Meetings, and other tasks as assigned by supervisor
- Ability to manage multiple priorities, and assess and adjust quickly to changing priorities
- Excellent interpersonal (verbal and written) communication skills are required to support working in environments that includes internal, and external customers
