With over 35 years of service, South Jersey Against AIDS remains the only AIDS Service Organization in southeastern New Jersey; a region of 1,500 square miles with over a million residents comprised by Atlantic, Cape May, Cumberland and Camden counties (Including three state prisons housing over 7,900 inmates).

SJAA’s challenge is to design and implement HIV care/treatment and prevention services that are based on the premise that all cultures are valued and that the strategies employed are culturally relevant. SJAA defines culture broadly to include populations and subpopulations to include IDU, MSM, transgender, HIV , women, and incarcerated. SJAA will dedicate resources to build knowledge, awareness, skills, and action plans to address a range of learning and to appreciate and benefit from diversity.
SJAA is committed to developing and implementing programs that will seek to integrate a broad range of services and intervention mixes that provide persons served the resources to improve their quality of life. SJAA will dynamically utilize creative approaches to HIV prevention of incorporating multi-level services that include: individualized outreach, interventions delivered to individuals, interventions delivered to groups and comprehensive risk counseling services, and referrals where appropriate.
Responsibilities:
- Conduct and complete a standard intake on any qualified client.
- Gather all pertinent documentation required by funding source.
- Explain thoroughly and have client sign all necessary releases, consents, and authorizations.
- With the client’s assistance conduct a needs assessment, acuity scale and develop a service care plan.
- Construct in appropriate order a client file including development of a unique case number.
- Accurately input case management service and referral details into agency database.
- Document all encounters with or for the client in the client file as detailed in the agency policies and procedures.
- Coordinate medical, dental and social service appointments.
- Facilitate service referrals for needed services in a timely manner.
- Complete and track medical and social service applications.
- Respond to emergency situations as they arise.
- Conduct routine follow-ups as dictated by the funding source.
- Generate monthly reports for agency as required
- Provide services to a diverse client population
- Attend all program meetings as required
- Routinely solicit client feedback
- Assumes additional responsibilities as assigned
- Ability to travel to other sites and locations that may not be easily accessed by public transportation.

Travel:
During the first 3-6 months all new hires will be required to complete federal, state, and agency training. This may result is colleagues traveling daily to additional sites. Travel beyond the colleague’s standard commute will be reimbursed monthly. Additional training may be needed beyond the first 6 months of employment. Training is a condition of employment with SJAA. Colleagues may also need to travel to attend SJAA meetings and/or functions such as our annual fundraiser.
Education/Experience:
Community Care Coordinator will have a minimum of an associate or baccalaureate degree in human services from an accredited college or university. Individuals without related academic degrees, relevant work experience conducted under the supervision of a human service professional or completed case management certification can be substituted. Case manager must have a valid New Jersey Driver’s License.
Community Care Coordinators will possess strong interpersonal skills, knowledge of cultural and lifestyle diversity issues, a working knowledge of HIV/AIDS and related issues, local healthcare entities, social service agencies and public entitlement programs.
Complete on-going state-approved basic HIV/AIDS training.
