Nurse Coordinator
South Jersey Behavioral Health Resources (SJBHR), a 501(c)(3) non- profit which may qualify for Public Service Loan Forgiveness is a community-based mental health organization dedicated to providing quality behavioral l health services in South Jersey since 1984.
SJBHR is currently seeking a full-time nurse coordinator (RN) to provide assigned consumers ongoing assessments of their psychiatric and medical condition. The RN will be responsible for conducting a comprehensive nursing assessment for each consumer admitted to the assigned program and complete a follow-up assessment every 90 days. The RN provides medication and medical education to consumer, families and other team members.
Assesses the medical needs of each consumer and coordinates care with the treating provider for appropriate team treatment.
Administers injectable medication to consumers. Routinely meets with residential counselors to provide supervision on documentation, implementation of service plan, and to address medical issues or concerns and provides assistance on medical devices consumer may be using.
Participates in agency-wide infection control committee and training of new employees on residential policies.
Coordinates consumer health information among all healthcare providers and maintains accurate and complete records.
Education and Experience:
RN must be licensed by the applicable New Jersey State professional board.
Valid drivers license is required.
Graduation from an accredited professional nursing program.
Two years of community mental health.
One year supervisory experience.
