What are the responsibilities and job description for the RN or LPN Triage Nurse position at South Lake Pediatrics?
Essential Duties and Responsibilities:
- Independently triage telephone calls from patients and parents making sure to effectively obtain concerns and symptoms with sufficient detail and accuracy to allow for appropriate assessment, classification, advice, and disposition of the patient/ parent’s request utilizing the appropriate South Lake Pediatrics protocols
- Management including assessment and appropriate routing and or follow up of patient documents and records
- Utilizing the nursing process, the triage nurse facilitates access to South Lake Pediatrics care system and/or develops an appropriate resolution to the patient’s/ parent’s concern
- Works closely with providers, nurses, floor staff, and other South Lake Pediatrics employees while demonstrating effective communication, collaboration, teaching, and negotiating skills, to build and maintain effective professional team relationships
- Prepare or complete prescription refills per South Lake Pediatrics Protocol
- Responsible to assist all clinicians with their daily patient correspondence, i.e. return phone calls, call in Rx’s, inform patients of the need for physicals and immunizations
- Provided time off from phones to complete delegated patient registry responsibilities
- Collaborate with and provide coverage for Care Coordination team as needed
- Additional tasks as assigned
- High school graduate or GED
- Must have current RN or LPN licensure
- 1 year phone triage experience preferred
Hours: 8-hour daytime shifts Monday - Friday
Clinic Hours are as follows:
Monday - Friday: 7:30am – 6:30pm M-TH, 7:30am - 4:45pm F (start time varies, and one late shift per week)
Saturday: 8:00am - 12:30pm (rotation: approx. once every 5 weeks)
Location: Hybrid position (Work from home three days/week, plus work from Minnetonka clinic two days/week)
