Scope & Purpose of Position:

The Older Adult Case Manager will work directly with our elderly guests in our emergency shelter to provide guidance and support for their aging journeys. The Older Adult Case Manager provides case management, housing search, reporting, and support services to guests in a low-barrier emergency shelter that promotes a Housing First philosophy. The primary responsibilities of the Older Adult Case Manager are to coordinate elder care between multiple service providers, develop both short- and long-term care plans, help manage medical and behavioral concerns, assist with transitional care, and complete ongoing assessments of aging life care needs. Other responsibilities are to assist elderly shelter guests with the transition to permanent housing and to support a safe physical environment that encourages positive guest interactions and self-stabilization. The Older Adult Case Manager is also responsible for maintaining appropriate data and records in addition to effectively communicating with the supervisor, staff, and other shelter guests in supporting and encouraging guests in achieving goals as outlined in their housing plans.

Essential Duties & Responsibilities:[1]

· Responsible for processing and welcoming new referrals during shift hours. Review policy & guidelines with guests upon arrival.

· Assesses guests’ barriers to permanent housing as well as other service needs. Assists guests in developing a plan to access community resources to re-stabilize housing, achieve permanency, and improve overall wellness.

· Facilitates linkages to all available and appropriate housing resources, including subsidized and market-rate housing, rental assistance, relocation funds, co-housing, nursing facilities and permanent supportive housing.

· Monitors shelter facility and promptly reports maintenance or repair needs. Assists in maintaining clean, safe, well-maintained grounds through regular observation of outside activity, debris removal, and reporting of any safety hazards.

· Case Mangers assist with the delivery of on-site, voluntary, no-cost programming and activities for guests either through internal or external resources.

· Assists guests in accessing material support for their basic needs, including food, clothing, medicine, and other amenities.

· Attend client medical appointments and provide transportation to appointments.

· Assesses guest’s needs and develops a care plan in order to coordinate specialty care to monitor health care needs.

· Assist with eligibility screening for assistance and entitlement programs.

· Assist with money management and guardianship issues.

· Secures guest medication and other valuables for safekeeping.

· Conducts 1:1 coaching, technical support, and/or no-cost, voluntary workshops or programming for guests as requested on topics including job readiness, landlord-tenant relations, and other pertinent subjects.

· Maintains guests’ files, and complete all internal communication, including incident reports and program logs in a timely manner.

· Helps train new program staff and supervises volunteers when assigned by the Executive Director and Deputy Director.

· Models and encourages professionalism and positive, strengths-based, client-centered, cooperative environment. Delivers services using principles of harm reduction and trauma-informed care.

· Supports smooth day-to-day functioning of the programs, including staff and resident communication; night shifts procedures; resolution of problems; receipt, storage, and tracking of donations; critical incident response and documentation; etc.

· Maintains strong working relationships with community partners, volunteers, and the public.

· Work with Deputy Director to assure compliance with funding requirements.

· Participates in on-call rotation shared with other Case Managers. Works flexible hours, which may include nights and weekends periodically to accommodate program and guest needs.

· Completes other assignments and tasks as needed and as directed by supervisor.

· Advocates for the mission of the organization and upholds agency values.

Qualifications:

· Bachelor’s degree or equivalent experience in social work, the social sciences, allied health, or related profession.

· Bilingual (Spanish/English) strongly preferred.

· Familiarity with Housing First principles and practices.

· A passion for serving and working with older adults.

· Knowledge of the aging process and journey.

· Familiarity with homelessness and related issues, including addictions, mental health/deinstitutionalization, trauma, domestic violence, and/or poverty.

· Cultural competency and facility working with populations from diverse racial, ethnic, linguistic, religious, cultural, and socioeconomic backgrounds.

· Prior lived experience of homelessness or housing instability a plus.

· Experience with case management and access to entitlements, workforce development/job training, medical and behavioral health treatment, and other collateral supports.

· Facility with data and reporting standards within a professional nonprofit environment.

· Availability and willingness to work nights and weekends and to assist other programs as needed.

· Collaborative, facilitative approach to working with guests and community partners.

· Ability to work in a fast-paced environment under pressure as part of a team.

· Familiarity with Microsoft Office suite.

Pay: $45,000.00 per year
