Job Title: Medical Assistant - Transition Care Management
Location: Hybrid (3 days in-office, 2 days remote). Will train onsite for probationary/training period - 90 days and then will transition into Hybrid schedule.
Job Summary: We are seeking a dedicated and experienced Medical Assistant to join our team, specializing in Transition Care Management (TCM). The ideal candidate will have three years of experience in TCM and will be responsible for supporting patients through their transition from hospital to home care. This role involves both clinical and administrative tasks, ensuring seamless patient care and coordination. Experience in value-based care is a plus but not required.
Key Responsibilities:
- Patient Coordination: Assist in the transition of patients from hospital to home care, ensuring all necessary medical and administrative tasks are completed. Scheduling patients for follow up visits and reaching out to physicians as needed.
- Clinical Support: Perform routine clinical tasks such as taking vital signs, administering medications, and assisting with patient examinations.
- Patient Education: Provide patients and their families with education on post-discharge care plans, medication management, and follow-up appointments.
- Documentation: Maintain accurate and up-to-date patient records, including medical histories, treatment plans, and follow-up care.
- Communication: Serve as a liaison between patients, healthcare providers, and insurance companies to facilitate smooth transitions and address any issues that arise.
- Remote Monitoring: Utilize telehealth tools to monitor patients' progress and provide remote support as needed.
- Administrative Tasks: Schedule appointments, manage patient files, and handle billing and insurance documentation.
- Other tasks as assigned.
Qualifications:
- Experience: Minimum of three years of experience in Transition Care Management.
- Education: Certified Medical Assistant (CMA) or equivalent certification.
- Skills: Strong communication and organizational skills, proficiency in electronic health records (EHR) systems, and the ability to work independently. Working with E-Clinical Works is a plus, but not required.
- Work Environment: Ability to work in a hybrid environment, with three days in the office and two days working from home. Must have a space at home that can be dedicated to office use. Must be able to work independently.
