Job Summary
We are seeking a highly skilled Care Coordinator to join our team. The successful candidate will possess excellent clinical knowledge and expertise in managing patient cases from admission through discharge.
Responsibilities
- Ensure that patients meet care criteria throughout their stay.
- Initiate authorization with network providers.
- Maintain knowledge of clinical information by reviewing documentation, participating in staffing, attending care conferences, and collaborating with the treatment team.
- Perform concurrent review with network providers to promote compliance and ensure quality for retrospective review.
- Liaise with physicians and other treatment team members to document all aspects of treatment.
- Lead the flow of information between levels of care by collaborating with admissions, clinical, and financial departments, as well as patients and their families/guardians.
- Coordinate resources with community services and agencies.
