Job Summary: Provides direct and indirect patient care in the ambulatory care setting. Provides care that meets the psycho-social, physical and general aspects of care; meets the communication needs of patient and family; provides care that reflects initiative and responsibility indicative of professional expectations, under the supervision of a clinical care manager. Maintains regulatory requirements, nursing and ambulatory care policies, procedures and standards.
Demonstrates Competency in the Following Areas:
- Provides direct patient care under supervision, evaluates outcomes, consults with other healthcare team members as required and adjusts nursing care processes as indicated to ensure optimal patient care.
- Ability to perform a head-to-toe assessment, under supervision, on all patients and reassessments as per policy. This includes neonate, pediatric, adolescent and geriatric patients and the general patient population.
- Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
- Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
- Communicates appropriately and clearly to team leader, coworkers and providers.
- Demonstrates the ability to assist providers with procedures and perform services requiring technical and manual skills.
- Demonstrates the ability to perform treatments and provide services to level of licensure.
- Demonstrates knowledge of lab requirements and requisitions.
- Maintain accurate counts of all medication dispensed through proper documentation.
- Triage medical calls and troubleshoots problems as they occur.
- Ensures all medical equipment is properly maintained and maintenance is documented.
- Demonstrates the ability to be flexible, organized and function under stressful situations.
- Demonstrates proficiency in phlebotomy.
- Performs other duties as assigned within the scope of practice.
Regulatory Requirements:
- Current LPN licensure in the State of Louisiana in good standing
- Current BLS certification
- Able to communicate effectively in English required; bilingual preferred
Other Skills:
- Basic computer knowledge required
- EMR experience highly desired
