What are the responsibilities and job description for the Care Coordination Specialist position at Southeast Florida Behavioral Health Network?
Position Summary:
This position will be responsible for improving the outcomes for adults and children whose behavioral health needs have not been met with the traditional treatment array and/or who have repeatedly not been engaged in services. This position will also champion trauma informed care and recovery-oriented system of care principles and standards to ensure persons served have a voice/choice, and overcome obstacles and barriers caused by cross-system gaps and dysfunction.
This position will be responsible for coaching the Care Coordinators across the provider network in collaboration with the SEFBHN (Southeast Florida Behavioral Health Network) Directors
Required Education/Experience:
- Bachelor’s degree in Human Services field; or master’s degree preferred.
- Three years’ experience working with individuals with mental illness and/or substance use disorders.
Essential Job Functions:
- Identify, through data surveillance and meetings with network detox facilities, adult stabilization units and hospitals, individuals eligible for Care Coordination based on the priority populations identified by SEFBHN and DCF.
- Track the progress of individuals served through the Coordination of Care (CoC) Module and provide outcome and output measures for Care Coordination reporting.
- Manage Care Coordination funds in network service provider contracts.
- Monitor waitlists, service needs, gaps and redirect resources as needed, within available resources.
- Develop diversion strategies including crisis planning to prevent individuals who can be effectively treated in the community from entering detox/crisis units, psychiatric hospitalization, or mental health residential treatment due to unmet behavioral health needs.
- Assist network service providers’ Care Coordinators with multi-disciplinary staffing and staffing for individuals identified as high utilizers, or priority population as requested.
- Develop partnerships and agreements with community partners to leverage resources and share data.
- Engage with Medicaid MMA plan representatives or insurance carriers’ utilization reviewers when applicable.
- Provide technical assistance to Network Service Providers and assist in eliminating system barriers.
- Work collaboratively within established systems to share best practices related to family engagement and strength-based recovery. This includes but is not limited to being a liaison to child welfare, specialty courts and re-entry services.
- Make use of a quality assurance process to review Care Coordination standards and utilization over time.
- Ensure medical necessity of treatment recommendations is met by requiring the use of LOCUS/CaLOCUS assessments for all adults served through Mental Health Care Coordination. Monitor provider adherence to fidelity.
- Monitor data input to the CoC module by Care Coordinators within the Network Service Providers.
