Qualivis : Attestation required confirming work eligibility in the US and that Agency Staff does have a completed and accurate I-9 form on file.
Candidate MUST bring Drivers License to first day. Name on Drivers License MUST match name on RN license. Candidate must live at least 90 miles from facility.
Must provide Social Security Card or CS must confirm / add last four digits of SSN in notes.
