What are the responsibilities and job description for the Peer Specialist position at Southeast Healthcare?
Unique opportunity for outreach and engagement in the University District. This position will work in tandem with a partner peer supporter to engage people who are homeless or living with addiction. Hours for the position are Monday through Friday, 11:30 a.m. to 8:00 p.m.
The Peer will have a lived experience and provide peer support and follow-up, determine needs of the patient, and set up supports to ensure safety and possible treatment.
Key Responsibilities:
Engage and build trust with individuals who are experiencing homelessness or addiction.
Provide peer support and follow-up services to ensure individuals’ safety and well-being.
Assess the needs of those served and connect them with appropriate resources and treatment options.
Work collaboratively with a partner peer supporter to deliver comprehensive outreach efforts.
Ensure proper documentation and reporting of services provided.
