What are the responsibilities and job description for the Family Resource Center (FRC) Universal Care Coordinator/Resource Specialist position at Southeast Louisiana AHEC?
The Southeast Louisiana Area Health Education Center is hiring a full time Universal Care Coordinator to provide telephonic care coordination services for the Bureau of Family Health’s Family Resource Center (FRC). The virtual center offers needs assessments, resource linkage, and system navigation services. The FRC provides evidence-based individualized support plans with families consent. The FRC services are open to pediatric providers, families of children/youth with special health needs, and families/children experiencing social related health needs statewide.
Universal Care Coordinator/Resource Specialist Duties:
· Provide resource and referral services for families/children/youth and pediatric providers.
· Develop evidence-based individualized support plans based on identified needs and family priorities when families elect this service.
· Coordinate and provide outreach to families identified through state benefit and surveillance programs per FRC protocols.
· Document and track all care coordination activities within the FRC data system per established protocols to ensure accurate data/records.
· In collaboration with the FRC Manager, implement continuous quality improvement strategies to ensure efficient workflows and relevant/up-to-date services.
· Support FRC service health provider engagement pilots to test and refine promising practices and strategies
· Attend weekly FRC huddles, elevate successes and opportunities for improvement.
· Update/revise family assessment tools and resource library at least annually to reflect current resource need trends and best practices.
· Participate/provide content expertise for workgroups or committees as assigned.
Location: Southeast, LA – routine in-state travel as needed for supervision, team meetings, and strategic planning
Salary: $43784-61298; commensurate with experience
Required Education and Experience:
BS/BA degree required, Master’s preferred (social services, nursing, allied health, public health or associated field). LPN/ADN or AD Allied Health with at least 4 years related experience.
Superior verbal and written communication skills – bilingual English/Spanish desired.
Superior critical thinking and problem solving skills.
