Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in coordinating patient care, ensuring that individuals receive the necessary services and support throughout their treatment journey. This position requires a strong understanding of medical knowlegde as well as the ability to navigate complex healthcare systems effectively.
- This position encompasses case management of patients in New Orleans and surrounding areas on the Southshore.

Responsibilities
- Coordinate with nursing and other discipline's diagnostic evaluations to assess patient needs and develop individualized care plans.
- Monitor patient progress and adjust care plans as needed based on ongoing assessments.
- Collaborate with healthcare providers in medical-surgical settings, including hospitals and clinics, to facilitate seamless patient transitions.
- Maintain accurate medical records and documentation related to patient care and case management activities including following up on lab results and obtaining orders as needed.
- Respond to inquiries, concerns and complaints from patients and their caregivers.
Skills
- Working knowledge of healthcare laws and regulations.
- Excellent patient service skills, demonstrating empathy and understanding toward diverse patient populations.
- Proficient in managing medical records with attention to detail and confidentiality. Must possess strong organizational skills.
- Ability to work collaboratively within a multidisciplinary team while advocating for patient needs.
- Must possess excellent communication skills and have working knowledge using computers. Experience with Kinnser-Wellsky is a plus.
Required Qualifications:
- 3 years’ nursing experience in a med-surg facility OR 2 years nursing experience plus at least 1 year in home health. Hospice experience is acceptable.
- **Case management experience highly preferred.**
- LPN degree is required
- Valid nursing license with reliable transportation.

Pay: $55,000.00 - $60,000.00 per year
