Requirements
Key Responsibilities:
- Deliver individualized and group education sessions on diabetes self-management, covering topics such as nutrition, medication adherence, blood glucose monitoring, and lifestyle modifications.
- Collaborate with physicians, dietitians, and other healthcare professionals to create and implement personalized care plans that address each patient’s unique needs.
- Assess patient needs, establish measurable goals, and monitor progress through consistent follow-up appointments.
- Stay informed about the latest advancements in diabetes care and treatment, sharing knowledge with patients, colleagues, and staff as appropriate.
- Ensure accurate documentation of patient interactions and care plans, adhering to HIPAA regulations and organizational policies.
- Engage in community outreach initiatives to promote diabetes awareness, prevention, and education within the Middle Georgia region.
Qualifications:
- Active license as a healthcare professional (e.g., RN, RD, PharmD, or MD).
- Certified Diabetes Care and Education Specialist (CDCES) certification required.
- Minimum 2 years of professional practice experience in a healthcare field.
- At least 1,000 hours of diabetes education experience (or working toward this requirement).
- Strong communication and teaching skills with a patient-centered approach.
- Knowledge of diabetes management tools and technology (e.g., CGMs, insulin pumps).
Salary : $75,000 - $95,000
