Requirements
Key Responsibilities:
- Develop, implement, and monitor programs aimed at improving performance on quality measures.
- Conduct outreach to patients with care gaps to ensure timely interventions.
- Coordinate with external providers to obtain necessary medical records and information.
- Act as the primary liaison between clinic lead physicians and the population health team.
- Participate actively in monthly quality review meetings to identify opportunities for improvement.
- Facilitate effective communication across multidisciplinary teams to promote a culture of quality care.
- Identify eligible patients for SEMG’s clinical programs, including care management and remote monitoring services.
- Assist patients through the enrollment process, ensuring they understand the benefits and services available.
- Analyze quality metrics and performance data to track progress and identify trends.
- Prepare and present reports to leadership and care teams to inform decision-making.
- Prioritize tasks based on overall team goals and patient needs.
- Develop and implement innovative strategies to drive quality outcomes in partnership with lead providers.
- Current Registered Nurse (RN) license.
- Bachelor's degree in Nursing (BSN) preferred.
- Previous experience in value-based care or quality outcomes is strongly preferred.
- Experience in primary care or outpatient settings is advantageous.
- Strong ability to function independently and prioritize effectively.
- Exceptional communication and interpersonal skills.
- Proficient in using electronic health records (EHR) and other healthcare technology platforms.
- Analytical skills to interpret data and implement improvements.
- Proactive, detail-oriented, and committed to healthcare quality and patient care.
- Ability to lift up to 50 pounds
- Ability to push or pull heavy objects using up to 50 pounds of force
- Ability to sit for extended periods of time
- Ability to stand for extended periods of time
- Ability to use fine motor skills to operate office equipment and/or machinery
- Ability to receive and comprehend instructions verbally and/or in writing
- Ability to use logical reasoning for simple and complex problem solving
- Ability to travel to multiple locations to support business needs as required
