COMMUNITY HEALTH WORKER

SUPERVISION RECEIVED

The Community Care Coordinator provides general direction and oversight.

SUPERVISORY AUTHORITY

None.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • SEWAAH employees are expected to model and foster SEWAAH’s core values: Compassion, Service, Integrity, Collaboration, Equity, and Community.
  • Identifies or contacts members of high-risk or otherwise targeted groups such as members of minority populations, low-income populations, pregnant women, or COVID-19-impacted individuals.
  • Assist clients in obtaining healthcare information or services and addressing barriers that may prevent access to care, such as using technology to access patient portals and telehealth for medical, behavioral health, and social service care or addressing language, transportation, or insurance needs.
  • Conduct assessments and screenings using approved methods to determine health-related social needs, depression, food and housing insecurities, healthcare and prescription uncertainties, etc.
  • Assist clients in meeting emergent needs such as food, shelter, clothing, prescription medications, etc.
  • Advises clients or community groups on improving general health, such as diet or exercise.
  • Conducts in-person visits to monitor a client’s progress or to assess their needs. Client contacts may be conducted in a clinic, a client’s dwellings, a public location, outdoors, or other type of setting. 
  • Transports or accompanies clients to scheduled health appointments or referral sites.
  • Attend case conferences and meetings to address the client's needs and report any updates. 
  • Advises clients or community groups on issues related to diverse types of risk prevention such as COVID-19, diagnostic screenings such as breast cancer, pap smears, glaucoma tests, or diabetes screenings.
  • Develop effective working relationships with local community health partners and co-workers.
  • Detailed documentation of each contact and interaction within electronic health records software.
  • Participate in quality improvement activities and peer review as requested.
  • Attend meetings or health fairs on behalf of SEWAAH as requested.

  • Use of computer and associated software such as Microsoft Office programs.
  • Ability to maintain courteous and cooperative professional relationships with coworkers, supervisors, other agencies, clients and customers, and the public. 
  • Capability to communicate clearly and effectively in verbal and written format. 
  • Maintain confidentiality, use good judgment, and work independently. 
  • Proficiency in seeking creative solutions and problem-solving.

  • Capacity to be empathetic and maintain a positive attitude.

  • Capability to work from home, as needed.

TOOLS AND EQUIPMENT

Computers and associated hardware and software, client monitoring systems, and personal protective equipment.

PHYSICAL DEMANDS

Sitting, standing, or walking for extended periods of time; stooping and kneeling occasionally; extended use of hands and fingers to grasp, feel, and perform job duties appropriately; hearing and speaking to exchange information, seeing to read complex information.

WORK ENVIRONMENT

Shared office environment or home-work environment, if needed. Travel as needed and as appropriate and safe to do so. Variable working hours. Possible exposure to hazardous drugs and or bloodborne pathogens.

Job Type: Full-time

Pay: $22.00 - $24.00 per hour

Expected hours: 32 per week
