Job Purpose
Community Support Teams (CST) Paraprofessional Professional is responsible for providing CST services according to CST service definition and supervision requirements stipulated in Rule 10A NCAC 27G .0104. The primary purpose of this position is to provide an intensive community-based rehabilitation team services that provides direct treatment and restorative interventions as well as case management
Major Duties and Responsibilities (Under the direction of the Team Lead and/or QP)
- Assist with crisis interventions 24/7/365 as indicated in the Individual’s crisis plan
- Assist with behavioral and substance use disorder treatment interventions
- Participating in the person-centered planning process and ongoing monthly plan updates
- Provides psychoeducation as indicated in the PCP
- Consult with identified medical (for example, primary care and psychiatric) and non-medical (for example, DSS, school, DOJ) providers, engage community and natural supports, and include input in the person- centered planning process
- Ensure linkage to the most clinically appropriate and effective services including arrangement for psychological and psychiatric evaluations
- Provide and coordinate behavioral health services and other interventions for the Individual
- Assist with the development of relapse prevention and disease management strategies
- Linkage and referral to formal and informal supports
- Monitoring and follow up
- Communicates the Individuals progress and the effectiveness of the strategies and interventions to the Team Lead as outlined in the PCP
- Participating in the team weekly case staffing meeting
- Assisting the Team Lead in monitoring and evaluating the effectiveness of interventions, as evidenced by symptom reduction and progress toward goals identified in the Person-Centered Plan
- Assisting the Team Lead in consulting with identified providers, engaging community and natural supports, and including their input in the person-centered planning process
- Assist Individual with housing search including engaging landlords to rent to the Individual and writing reasonable accommodation letters
- Assist with connecting Individual’s to financial and in-kind resources to set up and maintain their household
- Prevent and mitigate housing crises including being a point of contact for landlord concerns
- Assist with rehousing the Individual if they are no longer able to stay in their unit due to eviction or risk of eviction
- Assist in developing daily living skills to stabilize and maintain housing
- CST maintains an Individual-to-staff ratio of 12:1 with a team maximum of 48 individuals
- Maintain confidentiality and comply with governmental laws and regulations, as well as Company policies and procedures and all other duties and responsibilities as assigned per State and/or Federal Service Definition(s) and/or Clinical Policy 8A.
- Contribute to the team by completing any other duties and responsibilities as assigned by SEBHS Director(s), Manager(s), Supervisor(s), and/or other SEBHS Authorized staff.
Training Requirements:
- Core Training: *Must be provided prior to hire date*
- Current First-Aid and CPR
- Blood Borne Pathogens (BBP)
- Restrictive interventions Certifications.
Within 30 days of hire:
- Agency required in-house trainings
- CST Service Definition Training/Clinical Coverage Policy - 3 hours
- Crisis Response Training - 3 Hours
- CST Service Note Documentation Training – 3 Hours
Within 90 days of hire:
- Introduction Motivational Interviewing (MI) Training by a MINT Trainer (2 days) - 13 hours
- Permanent Supportive Housing Training – 15 Hours
- Basics of Psychiatric Rehabilitation and Functional Assessment – 3 Hours
- Trauma Informed Care – 3 Hours
- Annually: Follow up training and ongoing continuing education required for fidelity to chosen modality*** (If no requirements are designated by developers of that modality, a minimum of 10 hours of continuing education in components of the selected modality must be completed – 10 Hours (Modalities must be ONE of the following: Cognitive Behavioral Therapy, Trauma Focused Therapy, and Illness Management and Recovery (SAMHSA Toolkit)
Minimum Qualifications
CST Team Member must meet the requirements for a Paraprofessional Professional and have the skills, knowledge, and abilities required by the population and age to be served. Also, must have a minimum of one (1) year documented experience with MH/DD/SA population. CST service providers focused on substance abuse must include a Certified Clinical Addiction Specialist, or a Certified Substance Abuse Counselor Certification as defined by North Carolina guidelines.
Paraprofessional" within the mh/dd/sa system of care means an individual who, with the exception of staff providing respite services or personal care services, has a GED or high school diploma; or no GED or high school diploma to provide a mh/dd/sa service. Supervision shall be provided by a qualified professional or associate professional with the population served. The supervisor and the employee shall develop an individualized supervision plan upon hiring. The parties shall review the plan annually.

- Valid Driver’s License, Auto Insurance, and reliable transportation
- Criminal/DMV background check for the respective state or national if lived in another state, NC Healthcare Registry, Office of Inspector General and National Sex Offender.
- Available to accept random drug testing.
Working Conditions
The Paraprofessional Professional may be required to work within the facility, outside the facility, community, school, and/or within the Individual’s home. Flexible hours required. The Paraprofessional Professional & team shall provide “first responder” crisis response on a 24/7/365 basis to the Individual receiving CST service. Travel using personal vehicle will be necessary and will also require a DMV background check and proof of personal automobile insurance.

Pay: $14.00 - $16.00 per hour
