What are the responsibilities and job description for the CARE ADVOCATE position at Southern Area Agency On Aging?
CARE ADVOCATE - Must possess a broad range of knowledge, skills and abilities to coordinate the delivery of comprehensive services designed to meet the needs of older individuals. Duties include comprehensive in-person assessment interviews in the homes of clients, care plan development, service arrangement, follow-up / evaluation; and options counseling. Strong human relations skills; as well as strong oral and written communication skills are essential. Extensive knowledge of aging and disability issues, health and community resources and eligibility requirements for public benefits is preferred. Bachelor's degree in social work, human services or related field is desired. Proficient use of Microsoft Office products is required.
