Under the supervision of the clinic manager, the Perinatal Health Worker will be responsible for providing Health Education to all patients receiving prenatal education including those patients enrolled in the Comprehensive Perinatal Services Program.
JOB DESCRIPTION
- Responsible for implementation and integration of CPSP Program including nutrition, psychosocial, and health education assessments, interventions, and perinatal education with basic obstetrical care.
- Responsible for providing assessment, documentation and program eligibility for the CPSP program.
- Identify patients with positive pregnancy tests daily and determines CPSP Program eligibility.
- Perform outreach timely to offer CPSP services to qualifying patients either in person immediately after their medical appointments or by telephone.
- Coordinate development of a complete individualized care plan. Modifies the care plan as the client’s condition changes.
- Provide Health Education (Nutrition, Prenatal, and Psychosocial Services) to qualifying patients.
- Oversee the completion of all recommendations made on the care plan.
- Ensure that results of tests and referrals are given to appropriate team members and are recorded in the patient’s chart.
- Keep track of the patient’s attendance at appointments, identifies the reason for a missed appointment, an assist the client with making a new appointment.
- Document care provided in the patient’s chart timely, completely and correctly.
- Any other duties as assigned.
Education, Experience, & Qualifications :
- Bilingual (English/Spanish)
- 2 years work experience in a medical office
- High School Diploma(GED)
- CPSP certification and a minimum one year experience.
- Working knowledge of the CPSP program, educational materials and forms including "Steps to take" manual preferred.
- Medical Assistant Diploma-accredited vocational school
- Experience in operating a computer and word processing
- BLS/CPR-American Heart Associations
