What are the responsibilities and job description for the Wound Care LPN position at Southern Healthcare Management?
Responsible for managing the skin program for the center. Evaluates resident's skin and provides care and related treatment in accordance with physician orders and professional standards of practice.
- Evaluates the condition of residents' skin. Measures skin areas and documents in the resident's medical record. Initiates and updates care plan information regarding skin.
- Follows up with physician to report resident skin impairments that arise, and transcribes and follows up with new orders.
- Organizes and conducts wound rounds with center-specific wound consultant.
- Communicates with the physician, residents, family members, charge nurse, and other interdisciplinary team members, regarding resident status as appropriate.
- Maintains up-to-date and accurate wound care reports, utilizes computer programs available, and takes responsibility for the prevention and healing of skin impairments.
- Participates in Quality Assessment Performance Improvement (QAPI) activities as assigned.
- Collaborates with the interdisciplinary team, to assure accurate resident evaluation and development/revisions of individualized plans of care.
- Participates in weekly meetings to discuss residents with pressure injuries.
- Conducts rounds on units, with specific attention to residents at risk for or with skin integrity impairments.
- Attends in-service education programs and participates in Relias Training, as assigned, to learn new procedures, develop skills, etc.
- Performs other duties as assigned.
- Graduate of an accredited school of Nursing.
- Valid State licensure as an LPN
- Current Wound Care certification.
- Current CDC Infection Prevention certification.
- Current CPR certification.
SUPERVISORY RESPONSIBILITIESSupervises LPNs and certified nursing assistantsPHYSICAL DEMANDS AND ENVIRONMENTShift work may be required. Employee is frequently required to stand, walk, use hands or fingers, reach with hands and arms, feel, talk and hear. Employee will occasionally assist to lift and / or transfer patients weighing between 100 to 250 pounds. Employee is occasionally exposed to blood or other body fluids, fumes or airborne particles and toxic or caustic chemicals.
