Overview:
We are proud to expand our community impact in partnership with the Union Mission in Bluefield, WV, providing critical support through the Certified Community Behavioral Health Clinic (CCBHC) model. This new role will empower individuals by connecting them with essential behavioral health, social, medical, and educational resources, fostering positive change and stability in our community.
The Case Manager/Care Coordinator Level 2 plays a vital role in connecting consumers with essential services, including behavioral health, medical, social, educational, and other health-related resources. This position prioritizes consumer advocacy, effective care coordination, and the development of person-centered treatment plans. Support will be provided at the Bluefield Union Mission.

Qualifications:
- Bachelor’s or Master’s degree in a human services-related field, approved by the West Virginia Board of Social Work OR a licensed Psychologist with a Master’s or Doctoral degree OR a licensed Registered Nurse.
- Valid driver’s license and satisfactory driving record
