We want your knowledge and expertise!Job Summary and QualificationsThe behavioral health coordinator discharge planner is responsible for fulfilling the recommended discharge plan of the treatment team and clinical discharge planner. The behavioral health coordinator discharge planner will support the clinical discharge planner with patient communication, collaborating with the treatment team, and ensuring regulatory requirements are met. What qualifications you will need : Education : High school diploma - RequiredBachelor's degree - PreferredSkills : Ability to establish and maintain collaborative and effective working relationshipsAbility to communicate effectively in oral, written and electronic formatsDemonstrates critical thinking abilities with proactive decision-making and negotiation skillsExperience : One (1) year behavioral health experience. Case management / discharge planning experience - PreferredCertification : CPI certification within 30 days of hireDuties Include But Are Not Limited To : Provides discharge planning services for behavioral health inpatient unitEnsures patients designated for transition from one level of care / setting to another is completedEnsures identified discharge needs of patients / family are attained prior to discharge of the patientFulfills the discharge plan and collaborates with the treatment team and in particular with the clinical discharge planner to facilitate a successful care transitionCollaborates with the clinical discharge planner any barriers with the requested discharge plan Ensures discharge planning is completed timelyInvolves patient, family / responsible / significant others in identifying and clarifying needs and expectations to sustain a successful discharge plan post hospitalizationCommunicates with the clinical discharge planner and nursing to ensure patient throughput with an ongoing focus on an effective care transition, quality and efficiencyDocuments discharge plan and care coordination interventions to effectively communicate to all members of the health care teamAlign patient’s needs with available resources to ensure a safe discharge / transitionActs as a liaison through effective and professional communications between and with physicians, patient / family, hospital staff, and outside agenciesAdheres to established policy and procedure and standards of care; escalates issues through the established chain of command timelyDemonstrates knowledge of regulatory requirements, HCA Ethics and Compliance policies, and quality initiativesServe as an advocate for patient's rights, needs, and values; ensuring that patients’ ethnic, cultural, or religious values, beliefs, preferences and needs are considered and alignedEnsures regulatory requirements and third party payor requirements are met timelyMaintains current community resources to secure establish safe discharge planningExtrapolate information from electronic systems including MidasOther duties as assignedSouthern Hills Hospital is a 260 bed acute care hospital.
