We are seeking a qualified Patient Care Manager, RN to join our team, who shares our commitment to excellence and has the skills and experience necessary to excel in this role.
In this position, you will have the opportunity to work closely with our interdisciplinary staff to provide comprehensive care to our patients. You will be responsible for overseeing the supervision and coordination of clinical services, including developing and implementing individualized plans of care.
- You will ensure that patient needs are continually assessed and care rendered is individualized to patient needs, appropriate, and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
- You will oversee and assure development, implementation, and updates to the individualized patient plan of care, as appropriate.
- You will manage and document phone calls from physicians, clinicians, patients, referral sources, and communicate patient updates/new orders to clinicians. You will use coordination notes to document, as needed and appropriate.
- You will review assessments and plans of care daily, per assigned workflow, and consult clinicians with recommendations, as appropriate.
- You will coordinate all aspects of care with all disciplines, physicians, durable medical equipment providers, caregivers/family members, transferring facilities, and any other applicable healthcare providers.
