The Home Health Patient Care Manager plays a crucial role in supervising and coordinating clinical services to ensure high-quality care for home health patients.
- Receives referrals and assigns clinicians or therapists for timely patient evaluation, ensuring that the start of care (SOC) visits are properly authorized and plotted.
- Covers the determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
- Ensures that patient needs are continually assessed and care is individualized to meet those needs, while also being appropriate, reasonable, and compliant with home health eligibility criteria and physician orders.
- Oversees the development, implementation, and updates to the individualized patient plan of care as necessary.
- Manages and documents phone calls from physicians, clinicians, patients, referral sources, and communicates patient updates/new orders to clinicians using coordination notes when needed.
- Reviews assessments and plans of care daily, per assigned workflow, and consults clinicians with recommendations as required.
- Covering all aspects of care with disciplines, physicians, durable medical equipment providers, caregivers/family members, transferring facilities, and other applicable healthcare providers.
Requirements
- Current RN licensure in the state of practice.
- Current CPR certification required.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle.
