Position Summary
- The Diabetes Educator works collaboratively as an active member of the interprofessional team to provide therapeutic patient and family centered care in the inpatient and outpatient settings. This position will provide education to individuals and families on a wide variety of topics with an emphasis on diabetes education. The Educator systematically and continuously collects and assesses data related to patient health status to develop, execute and evaluate the plan of care. This position utilizes evidence to drive clinical decisions and practice related to individual and population health. The Diabetes Educator establishes goals and strategies for meeting care coordination needs across settings with the patient, family, and/or other care providers. The educator considers factors related to safety, effectiveness and fiscal accountability in planning and delivering care. The educator participates in continuous quality improvement initiatives and assumes responsibility for their professional development.

Education
- Associate degree in Nursing or Bachelor of Science in Human Nutrition and Dietetics
Licenses and Certification
- 15 contacts hours of approved diabetes related continuing education annually
- Current Illinois RN License, CDCES or willingness to obtain within 2 years of hire
- Current Illinois RD License with current CDCES certification
Experience and Skills
Technical Experience: 3 years in related field
Role Specific Responsibilities
- Collects DSMES program participant assessment data in a collaborative and ongoing manner.
- Collaboratively develops educational goals, learning objectives and a plan for educational content and teaching methods with DSMES program participants.
- Provides educational interventions that utilize primarily interactive, collaborative, skills-based training methods and maximizes the use of interactive training methods
- Collaboratively develops an individualized follow-up plan with each program participant.
- Evaluates effectiveness of educational services provided by measuring attainment of learning objectives.
- Conducts a follow-up assessment upon completion of DSMES program services, using outcome measures.
- Documents assessment data, educational plan, educational services provided and evaluation results in each participant’s educational records.
- Utilizes a team approach to provide services and collaborates and communicates with team members when needed
- Participates in the development of training materials.
- Contributes to, and participates in, a continuous quality improvement process to measure DSMES program and to identify and address opportunities for improvement
- Participates in planning and developing nutrition and diabetes management related policies, procedures, and goals.
- Participates in programs to meet continuing education requirements.
- Other duties as assigned.
Compensation (Commensurate with experience):
