INTRODUCTION OF CORE PRIVILEGES
Behavioral Health providers at SICHC request privileges to practice therapeutic counseling based on their training and experience. The categories and core privileges listed below are based on standards for Graduate Education in Social Work.
To be eligible to apply for LSW / LCSW privileges, the applicant must meet the following qualifications :
Demonstrate successful completion of an approved, specialized course of study which teaches the expanded clinical social work skills and knowledge relative to persons in the area of primary health care.
Must meet SICHC credentialing criteria.
Show proof of current license in the State of Indiana as a Clinical Social Worker.
Must hold Indiana Association of Social Work Board Certification (ASWB)
MEDICAL CHARTING RESPONSIBILITIES :
Documentation must be clear, legible, complete, and done in a timely fashion, describing each service the LSW / LSCW provides to a patient in SICHC and relevant observations. Standard rules regarding authentication of, necessary content of, and required time frames for preparing and completing the medical record and portions thereof are applicable to all entries made.
AUTOMATIC SUSPENSION OF PRIVILEGES :
If at any time, the LSW / LCSW’s employment terminates with SICHC, the LSW / LCSW’s privileges will automatically be suspended or if the LSW / LCSW is in the application process the application process will be suspended as well. The LSW / LCSW will have sixty (60) days to submit a request to the Credentialing / Privileging Department for modification.

Core privileges : All duties within the scope of the practice of a LSW / LCWS; apply social work theory and methods to the diagnosis, treatment and prevention of psychological dysfunction, disability or impairment, including emotional and mental disorders; and determine behavioral health diagnosis using the diagnostic taxonomies commonly accepted across disciplines among behavioral health professionals.

To be eligible to apply for special non-core privileges, the applicant must have documented training and / or experience and current competence in performing the requested procedure(s) consistent with criteria set forth in medical staff policies governing the exercise of specific privileges. This may be accomplished by providing documentation of acceptable supervised training and experience or successful completion of an approved, recognized course when such exists.
